Gaze-evoked nystagmus (GEN) due to a focal brain lesion is usually caused by a failure of the neural integrator, which was known to be localized in the interstitial nucleus of Cajal in the upper midbrain for vertical gaze holding and in the medial vestibular nucleus and nucleus prepositus hypoglossi in the upper medulla for horizontal gaze holding.
